Make it possible to iterate on plan mode

The current implementation of plan mode is not great.
It is impossible to iterate on the plan and ask questions.
Codex would start asking questions like: Do you want me to answer in short or a long answer?
It doesn't recognise conversation vs plan writing
It should be able to respond and reason based on new information added, and synthesize the plan instead of just rewriting it on every single request.
It only writes the plan on the latest information, not what happened in the same session.
