Context does not seem to update effectively using codex-mini-latest

What steps can reproduce the bug?

"Review all AGENTS.md files in the repo and add them to your context".

There are c10 files with significant text. Despite this, Context stays at 100%.

I have checked the last three releases and the behaviour remains consistent.

What is the expected behavior?

o4-mini-latest displays the same behaviour in the current release but in version 0.1.2505172116 (at least) the context drops to c90% post this request.
