What issue are you seeing?
There’s a bug in mode switching. Sometimes, when I talk to Codex, it thinks it switched to Plan mode, but it actually stays in Code mode. Because of that, it refuses to do certain things. If I switch the mode to Plan mode, I have to talk to it again so it realizes the mode changed. Then I need to switch the mode again and talk to it again so it can finally work. This happened multiple times today, very annoying.
What steps can reproduce the bug?
observe codex in code mode thinking it's in plan mode

I'm working on a PR rn to improve the code mode prompt
Also it would be helpful if you could run /feedback and paste the thread ID (session ID) here
My impression is that it started happening today after I updated the Codex CLI.
