What issue are you seeing?

When Codex CLI and Codex App need approval to run a command (e.g. psql), it will ask me for approval with three options:

1. Approve
2. Yes, and don't ask again for commands that start with psql
3. No

No matter how many times I select option 2: don't ask again, it will always ask for my permission to run that command. It's always asking permission on the same thing too - not a variation of the command or a different command entirely - that same "commands that start with psql" message. And it doesn't remember my choice.

What steps can reproduce the bug?

It remembers the choice and doesn't ask me again.
