App gets stuck on approval selection

What issue are you seeing?

Sometimes when the agent asks you to approve a command with escalated permissions it seems to be reading a file simultaneously and then when you try and select any approval option it doesnt work and also the agent never finishes reading the file.

What steps can reproduce the bug?

I'm really not sure, only that it seems to happen when codex tries to do 2 things at once. session id: 019c2f88-5c2c-72d1-8e81-7b508f3afdb9
