What issue are you seeing?
Codex CLI has a bug with Plan Mode.
From my experience, sometimes the model starts implementing while I’m still in Plan Mode. Then, even after Plan Mode disappears in the CLI (for example, after the model suggests implementation and I accept), it still keeps returning plans. When I tell it I’m no longer in Plan Mode, the model says it’s still in Plan Mode.
This bug means what we see in the CLI and what the model sees are different and out of sync.

@looper200 can you run
/feedbackand post the session ID?@charley-oai I lost the session where I encountered the bug. I managed to work around it by toggling Plan mode multiple times, which resets the behavior, but the bug still occurs occasionally.
I have just came across this issue myself with 0.99.0
It was able to modify the files in plan mode which isn't nice since I tend to use plan mode as a read-only mode. Cycling to default and back to plan mode didn't help, and the agent always said it is in plan mode no matter what I switched to via shift+tab.
Restarting codex cli worked. It was running on Linux / WSL.
