What issue are you seeing?

Codex CLI has a bug with Plan Mode.

From my experience, sometimes the model starts implementing while I’m still in Plan Mode. Then, even after Plan Mode disappears in the CLI (for example, after the model suggests implementation and I accept), it still keeps returning plans. When I tell it I’m no longer in Plan Mode, the model says it’s still in Plan Mode.

This bug means what we see in the CLI and what the model sees are different and out of sync.
