Codex VSCode extension stays stuck submitting Yes/No for permissions

What issue are you seeing?

Codex stays stuck "Thinking", awaiting for such confirmation, that I can not click

What steps can reproduce the bug?

Nothing particular, I can click it, it will work 2-3 times, and then suddenly starts failing. It started failing this morning, maybe then due to 0.4.71

What is the expected behavior?

I see this when I click "Yes", in the codex extensions logs :
