Enable clarifying questions in normal mode (not only Plan Mode)

Factory Droid is an agent harness sitting on top of Terminal Bench 2.0 with multiple models. One of the reasons the harness works well is that it proactively asks clarifying questions when a task is underspecified or ambiguous, which reduces bad assumptions and improves outcomes on complex tasks (including ones similar to Terminal Bench–style workflows).

Codex already demonstrates this behavior in Plan Mode (it can ask questions to narrow scope), but in normal mode it rarely—if ever—asks clarifying questions even when key constraints are missing. Please extend the same behavior to normal mode, with simple rules for when Codex should ask vs. make reasonable assumptions.
