Codex VS Code extension stopped working overnight with ChatGPT login (Plus user)

What issue are you seeing?

I’m a ChatGPT Plus subscriber.

Codex was working normally in the VS Code extension late last night.
I did not change any local configuration, plugins, or network settings.

After waking up, Codex via official ChatGPT login consistently fails
with the error: “stream disconnected before completion”.

On the same machine and same plugin version:

• Codex works via a custom provider
• Codex works on the web
• Codex does NOT work via official ChatGPT login in the VS Code extension

This appears to be an overnight regression without any client-side changes.

What steps can reproduce the bug?

1. Open VS Code
2. Sign in to Codex using official ChatGPT login (Plus account)
3. Open any project
4. Send any prompt (even a simple “hi”)
5. Codex fails with “stream disconnected before completion”

No local changes were made between the last successful usage and the failure.
