Codex App often fails to generate Pull Request title and summary

Often I've been getting the following error when generating pull requests from chatgpt.com/codex. This seems to be triggered for larger changes, although I haven't been able to pinpoint the exact cause for this based on observation only.

PR Title: Codex-generated pull request
PR Summary: "Codex generated this pull request, but encountered an unexpected error after generation. This is a placeholder PR message."

What steps can reproduce the bug?

1. Start a task in chatgpt.com/codex
2. Click "Open pull request" once change is finished
3. Go to github.com and observe the issue

What is the expected behavior?

PR summary and title are correctly generated according to change details.
