Failed to resume task when logged in – cannot start new agents

What issue are you seeing?

Creating a new agent while logged in fails with:

failed to resume task

Existing agents/tasks also fail to resume after restarting the IDE.

Starting an agent while logged out works, and logging in afterwards allows the agent to continue.

What steps can reproduce the bug?

1. Start VS Code / Cursor with Codex extension installed
2. Log in
3. Try to create a new agent
4. Error: failed to resume task
5. Restart IDE
6. Existing agents also fail to resume

What is the expected behavior?

New agents should start normally and existing agents should resume after IDE restart.

Additional information

Occurs on multiple machines.
Extension reinstall and storage reset did not help.
Downgrading extension versions also did not fix the issue.
