What issue are you seeing?

I created a PRD.md file in the project root directory and then switched to worktree mode in codex app, asking the agent to implement it.
The agent searched for a long time but couldn't find the file, and finally gave up.
The session jsonl (renamed to json) is attached.

What steps can reproduce the bug?

1. clone codex repo
2. create an md in the root dir
3. switch to worktree mode, @ the file and let it implement it

What is the expected behavior?

The agent can find and read the file
