Support the `/approval` command

Supporting /approval to change the user's approval_policy setting is comparable to what we need to support /model as described in https://github.com/openai/codex/issues/1251, though arguably it is a bit simpler because we should be able to change the approval_policy on the fly without asking the user to restart their conversation.
