"Quick Switcher" Missing from `View` Menu in macOS Menu Bar

What issue are you seeing?

The "Quick Switcher", toggled by the shortcut CMD-k, is missing from the macOS Menu Bar. Specifically, the View menu from which it is invoked.

What steps can reproduce the bug?

• Open Codex.app on macOS
• Ensure Codex app is the active application by checking the Menu Bar for "Codex" in bold font
• Use the keyboard to press and hold the Command (CMD) key
• While holding the Command (CMD) key, press and release "k"
• Notice the "Quick Switcher" feature is present and has been invoked
• Also Notice the brief flash on the macOS Menu Bar indicating the invocation was through the "View" Menu
• Move your cursor to the macOS menu bar, specifically over the "View" Menu Button.
• Click and release the "View" Menu Button to open the "View" Menu.
• Notice that there are many commands listed.
• Notice that there is no listing with for a command registered to CMD-k

What is the expected behavior?

It is expected that any keyboard shortcuts implemented by a macOS application be properly registered, including a listing within the appropriate menu (such as the "View", or "Edit" menus) of the macOS Menu Bar.

Meeting this expected behavior would

• Improve user experience
• Increase discoverability of features
• Improve developer experience
• Reduce erroneous "missing feature" reports from users
