"Quick Switcher" fails to dismiss via the invoking keyboard shortcut `CMD-k` on macOS

What issue are you seeing?
The "Quick Switcher" View fails to dismiss via CMD-k after invoking it with CMD-k.
What steps can reproduce the bug?
- Open Codex.app on macOS
- Ensure Codex app is the active application by checking the Menu Bar for "Codex" in bold font
- Use the keyboard to press and hold the Command (CMD) key
- While holding the Command (CMD) key, press and release "k"
- Notice the "Quick Switcher" feature is present as a modal view, and has been invoked
- Repeat steps 3-4
- Notice the "Quick Switcher" modal view is still present, and has not been dismissed
What is the expected behavior?
It is expected that a modal view presented by a dedicated "toggle" keyboard shortcut be dismissed by that same keyboard shortcut.
Meeting this expected behavior would
- Improve user experience
- Streamline App navigation
- Reduce unnecessary friction for keyboard users
- Improve compliance and Reduce Risk
- Increase the accessibility, or A11y, of the App
- Meet system-wide macOS user expectations for "modal views"
