What issue are you seeing?

Title: Codex got stuck waiting for permission but never prompted for approval

Description:

While using Codex, it entered a pending state waiting for permission, but no approval prompt was shown.

Expected behavior:

If Codex requires permission, it should explicitly prompt for approval.
If permissions are updated to Full Access, the task should automatically resume or re-request approval properly.

Actual behavior:

Codex remained stuck in a pending state without prompting for approval.

What steps can reproduce the bug?

I asked for a plan, codex created the plan, then I approved the plan, codex started to do and spawned sub agent, so that happened

What is the expected behavior?

Ask for a permission to me
