Plan mode (switch & chip) in Codex App is buggy

In a new chat thread > Press Shift+Tab to Turn on Plan mode > The 'plan mode' chip is visible/hidden
After chat is kicked off,
ISSUE#1 : Shift+Tab doesn't reliably turn on/off the plan mode switch in "Add files and more" drawer(?).
ISSUE#2: Even if plan mode switch is toggled, the plan mode chip in chat box doesn't show/hide automatically.
