Plan mode (switch & chip) in Codex App is buggy

In a new chat thread > Press Shift+Tab to Turn on Plan mode > The 'plan mode' chip is visible/hidden
After chat is kicked off,
ISSUE#1 : Shift+Tab doesn't reliably turn on/off the plan mode switch in "Add files and more" drawer(?).
ISSUE#2: Even if plan mode switch is toggled, the plan mode chip in chat box doesn't show/hide automatically.

I believe I'm having a similar issue but I've also noticed that the plan mode indicator shows after you switch to a different application and then switch back. So maybe it is toggling plan mode on and off but the indicator is broken?

I also notice that if you interject the plan mode making a plan, the 'Plan mode' chip message at the bottom right turns off. But the model sees it still as engaged, and you can't actually accept the plan.
Making a new plan fixes it.
