This is the first report we've received of this issue. Let's see if we can determine whether it's a regression in the extension or something that changed on your system (e.g. a corrupt file or configuration). Could you try downgrading to the previous version of the extension to see if the problem goes away?

I have tried downgrading the extension to the previous version as well as the one before that. I also completely uninstalled VS Code and Codex, and removed all related files from my computer. Unfortunately, nothing has worked so far.
I started troubleshooting this issue yesterday and have also sought help from Gemini and Perplexity, but I haven’t been able to resolve it.

OK, that confirms it's not a regression in the extension and is likely caused by something that changed on your system.
The next thing I'd recommend is temporarily renaming the ~/.codex directory and relaunching the extension. It will create a new ~/.codex and eliminate any non-default config settings and old state.
