What issue are you seeing?

Was trying to /feedback for the endless, constant permissions requests (tried default, full permissions, "for this session" does not do anything) and was unable to do so without being constantly interrupted by the permissions requests.

This is pretty dangerous, count the number of A's in this message and consider each of those inadvertently giving codex "always" permissions without even seeing what it asked for (and no way to review)

I tried asking codex to figure out what is going on and it concluded everything is broken and I should wait for y'all to fix it. This somehow cost me multiple percentage of my weekly usage

What steps can reproduce the bug?

Use codex as normal

What is the expected behavior?

It works without consuming usage at hyperspeed or asking permissions for every single thing it does regardless of what is selected or configured

Additional information

This is causing reputational harm to the product. I am approaching the point of buyer's remorse
