Dictation stops (and appears to cancel) when command approval prompt appears during a session

What version of the Codex App are you using (From “About Codex” dialog)?

26.303.1606

What subscription do you have?

Pro

What platform is your computer?

Darwin 25.3.0 arm64 arm

What issue are you seeing?

When using voice dictation in the Codex app, if a “command approval” prompt appears (e.g., approve running a command), dictation immediately stops. It seems to quit/cancel the dictation session rather than pausing it or finishing it.

This is especially disruptive if I’ve been dictating for a while: I have to restart dictation. This has been giving me a headache when I have been dictating for multiple minutes and essentially lose the progress.

What steps can reproduce the bug?

  1. Run a prompt that will trigger an approval prompt
  2. While it is running, start dictating
  3. When it shows that I need to approve, the input box disappears, and the dictation has stopped

What is the expected behavior?
