Clickable links don't work right

What issue are you seeing?

Hover over on a text file link shows the correct file path, but clicking the link opens a terminal at the project root.

What steps can reproduce the bug?

Generate a response containing a file path link, click the link.

What is the expected behavior?

The file opens in the default app except for text files which should open in the codex-app-configured text editor.
