Mentioning codex on PR's sometimes does not trigger cloud code reviews. Sometimes takes 3-4 @ mentions in a row over the course of 10 min for it to trigger.

What steps can reproduce the bug?

I have experienced a similar phenomenon to this issue.
I have pro subscription and usage limit is enough.

When I try to start a task using the @codex command as a github issue comment, codex cloud sometimes ignores some requests. (The bot doesn't even attach :eyes: emoji)
This phenomenon occurs especially frequently when multiple @codex comments are added in a short time.

For example, when I added three @codex comments at once using github workflow, they were ignored.
To avoid this, I monitored the eyes emoji and then added the next @codex comment in the workflow but the second comment wasn't recognized by codex.

I have the same issue as well. Codex review is enabled for the repo as verified on every push, I tagged codex I see no response from it even after waiting for a long time.

Tried with claude action in the same repo - it works
