It seems this issue occurs by using VPN... I turned off and works fine. But shouldn't be doing that.

I've been getting this issues for several days now. Reported multiple times via Feedback in Codex app, the latest being 019d1fff-5705-7092-9468-30adf07dfbdc

Same. Running 5.4 (fast) xhigh in codex app macos. Context window was 88% full. I did have VPN on and it failed with this error 3 times. I just switched VPN off and now it's working, similar result to what another user reported above.

Thank you @gwest1000 I've just turned off my vpn and it has worked again. I've had the VPN on all day and compacting has occurred multiple times, but the issue has happened a couple of times today. Strange but working now, cheers

try this skill, https://github.com/haibindev/golast, It recovers the last actionable request from a crashed or compacted session, so you can pick up exactly where you left off.

Turning off VPN has been consistently working for me but this is obviously not a fix, as I and many others prefer to have VPN on generally. I'm getting this error daily, and it's a pain.
