What issue are you seeing?

Title:
Codex Windows app from Microsoft Store crashes on launch with code 3221225781 after blank loading window

Body:
I installed the Codex Windows app from Microsoft Store on Windows 11.

Behavior:

  1. Codex opens a blank/gray loading window and stays there.
  2. If I wait or close/reopen, I get this error:

Codex app-server exited unexpectedly (code=3221225781, signal=null)
Most recent error: None

What I already tried:

  • Removed/renamed config.toml
  • Reinstalled the app
  • Reset the app from Windows settings
  • Checked WebView2 runtime (installer says it is already installed)
  • Reliability Monitor only shows successful Codex Store install/update entries
  • The app still fails before it becomes usable

Notes:

  • The error dialog mentions “extension”, but this is the Microsoft Store Windows app, not a VS Code extension.
  • After Reset, the app briefly changed behavior: first a blank loading window appeared, then later the same crash returned.

System:

  • Windows 11
  • Codex installed from Microsoft Store

Please let me know what logs or diagnostics would help.

What steps can reproduce the bug?

  1. Install Codex from Microsoft Store on Windows 11.
  2. Launch the app.
  3. A blank/gray loading window appears and hangs.
  4. After waiting or reopening, the app shows:

"Codex app-server exited unexpectedly (code=3221225781, signal=null)"

What is the expected behavior?

The Codex app should open normally and become usable instead of hanging on a blank window or crashing.

Additional information

I already tried:

  • renaming/removing config.toml
  • reinstalling the app
  • resetting the app in Windows
  • checking WebView2 (installer says it is already installed)

This is the Microsoft Store Windows app, not a VS Code extension.
