Cyber Verification Failed – cyber_verification_precheck_failed Error and Context Issues with GPT-5.4

I previously completed the cyber verification process, but it appears that the verification has been revoked.

When I try to run the verification again, the following error occurs:

I am currently using GPT-5.4. The main issue is that once the context exceeds 256k, the model no longer seems to properly understand my queries.

This behavior makes me suspect that the issue may be related to my cyber verification not being properly applied to my account.

Could you please check why my account is not eligible for cyber verification and why the verification appears to have been removed?
