Accessible diff markers for added and removed lines in Codex App

What feature would you like to see?

I would like Codex App to expose added and removed lines in diffs more accessibly. Right now I do not reliably get clear + and - markers in a way that is easy to understand with a screen reader, so it is difficult for me to tell what was added and what was removed.

I am a blind user, so diff readability is not just a convenience issue for me. I need the diff view to clearly communicate additions and deletions in text, in a way that works well with screen readers and keyboard-based review.

Additional information

This would make code review and change verification much more accessible in Codex App. Ideally, the diff should have a clear textual representation of added and removed lines, not only a visual presentation that depends on sight.

If possible, it would also help to have an accessibility-friendly mode for diffs where additions and deletions are announced explicitly and consistently.
