v0.116 repeatedly prompts for escalation on same file despite "don't ask again" (works in v0.114)

What issue are you seeing?

  • Repeated prompt appears with reason: command failed; retry without sandbox?
  • Selecting "don't ask again for these files" does not suppress next prompt on same file.

What steps can reproduce the bug?

v0.116:

  1. Ask Codex to create test.md with one "Lorem ipsum" line.
  2. Approval dialog appears; choose 2) Yes, and don't ask again for these files (a).
  3. Ask Codex to edit same file (test.md) and append another line.
  4. Approval dialog appears again for same file.

Same flow on 0.114.0 updates file without repeated escalation prompts.

What is the expected behavior?

codex should updates file(s) without repeated escalation prompts.

Codex (after asking to analyze the issue):

  bwrap: loopback: Failed RTM_NEWADDR: Operation not permitted
