Codex VS Code extension gets stuck on “Thinking”

What issue are you seeing?

When I send a prompt in the Codex VS Code extension, it often stays in the “Thinking” state and does not complete successfully. Just Thinking and sometimes, stream error.

I initially suspected a regression in the latest extension version, but I tested multiple older versions as well and saw the same behavior.
