Codex extension repeatedly ask for permission

What issue are you seeing?

Codex reports a broken sandbox and repeatedly ask permissions even if the edits happen locally in the workspace. "Don't ask again" simply makes no difference.

I use codex intensively everyday, and this issue pops up in 1 or 2 days. I always use default permission, GPT5.4 with extra high reasoning efforts.

What steps can reproduce the bug?

ask codex to edit any file or create a new file in the workspace.
