What version of the Codex App are you using (From “About Codex” dialog)?

26.318.11754 (1100)

What subscription do you have?

Plus plan

What platform is your computer?

Darwin 25.3.0 arm64 arm

What issue are you seeing?

Threads in the Codex app randomly disappear from the thread list/history without any explicit delete action.

A thread can be present initially and later no longer appear. This makes it impossible to recover previous conversations/context.

Expected: threads persist unless explicitly deleted by the user.

Threads stay in Codex CLI history and can be resumed with /resume
Potentially related to local cache or sync inconsistencies.

What steps can reproduce the bug?

  1. Create/use multiple threads in Codex App.
  2. Continue using the app and/or restart it.
  3. Observe that previously visible threads are no longer listed.

Threads stay in Codex CLI history and can be resumed with /resume

Note: no delete action was performed.

What is the expected behavior?

Thread history should persist and remain visible unless the user explicitly deletes a thread. Restarting the app or syncing should not remove threads.
