Codex App repeatedly fails with "timeout waiting for child process to exit" after update

What issue are you seeing?

After updating Codex App on macOS, I repeatedly get the error:

timeout waiting for child process to exit

The app works for a while, then starts failing with this error again.
Restarting the app does not reliably fix it.
Starting a new thread also does not help.

What steps can reproduce the bug?

1. Open Codex App on macOS
2. Use it normally for a while
3. After some time, the app starts failing with:

timeout waiting for child process to exit

What is the expected behavior?

I expected Codex App to continue working normally without timing out.
