What issue are you seeing?

I am granting permission to "codex" to operate on certain files, but "codex" does not remember that (even inside the same session) and keeps asking the same permission over and over again

What steps can reproduce the bug?

also, today I tried again, here are two screenshots , where I grant permission to operate on a file, twice
