What issue are you seeing?

The approval is not working anymore, using Permissions: Custom (workspace-write, on-request)
I have to approve every little modification, the "remember" approval feature doesn't work.. It forces me to make /approval and choose full. cause doens't work anymore.

What steps can reproduce the bug?

If I open the cli and any modification is needed i have to approve again..
