clipaste fixes this. Tiny background daemon that normalizes the screenshot clipboard — adds a file URL so Cmd+V works, and adds the PNGf type so Ctrl+V works.

brew install hqhq1025/clipaste/clipaste
brew services start clipaste

Also supports SSH remote paste via clipaste ssh-setup user@host.

Also seeing this on macOS.

When I copy a screenshot/image to the clipboard and paste it into Codex, the image does not attach. The same screenshot paste flow works in Claude Code and OpenCode.

This seems to be the same clipboard image-paste problem, just reproduced on macOS as well.

I have a similar but slightly different issue - the image gets recognized in the codex chat as [Image #1] but it's the wrong image - instead of the file I copied, it's s generic system image icon like this:

<img width="1424" height="1738" alt="Image" src="https://github.com/user-attachments/assets/be1dc263-392c-4a24-ad61-539593801912" />

iTerm2 and Terminal.app
codex-cli 0.142.5
MacOS Tahoe 26.5.1
