Subagent UI "Open" button not working

What issue are you seeing?

The subagents say they're waiting for instruction, but the white "Open" link/button next to them doesn't seem to do anything. The coordinator (GPT-5.4 Extra High) doesn't seem to be fielding their questions very well either.

What steps can reproduce the bug?

1. ask it to launch subagents
2. try to step into each subagents and attend to its user-facing issues

What is the expected behavior?
