Subagent UI "Open" button not working

What issue are you seeing?
The subagents say they're waiting for instruction, but the white "Open" link/button next to them doesn't seem to do anything. The coordinator (GPT-5.4 Extra High) doesn't seem to be fielding their questions very well either.
What steps can reproduce the bug?
- ask it to launch subagents
- try to step into each subagents and attend to its user-facing issues
What is the expected behavior?

wish i still had the screenshot of the UI, but accidentally overwrite clipboard with uname command for platform version answer above.

i know there's often a delay when you click to review diffs (where a new tab will open in VS Code) but in this case even after a minute of waiting, no such tab(s) open
seeing same error as #16850 but still, it's different behavior (subagents, and diff review still works without this stream of errors) so I want to keep it open
This issue is also present on MacOS - seems to be specific to the extension. Codex CLI
/agentcommand works just fine AFAIK
