VSCode Codex UI files changed spam making chat unreadable

What issue are you seeing?

The files changed summary spawned additional elements while scrolling the conversation and now they won't go away.

What steps can reproduce the bug?

Scroll up and down while its generating chat messages. Maybe switch tabs and come back to the scrolling conversation. Not 100% sure.

What is the expected behavior?

A single files changed bar at the bottom; no duplicates.
