codex hallucinates by ending the turn responding to previous turn instructions

What version of the Codex App are you using (From “About Codex” dialog)?

What issue are you seeing?

please see attached chat. codex hallucinates/ gets stuck, responding to instructions in PRIOR turns (not the current one) . Clicking edit for the current turn to change the instructions does not get him out of this. This happens quite often , almost every chat at some point after it accumulates a certain number of turns .

note, codex STARTS the turn responding and working on things i asked him to the current turn. it is only when work is completed, that the work portion is collapsed and he begins summarizing, that he jumps back aaddressing a prior turn.

In the chat log, see the last turn and penultimate turns. it is evident that he made a commit -- which i asked him to in penuiltimate turn, followed by new instructions in the last turn -- however both penultimate and last turn he comes back about making the commit.

What steps can reproduce the bug?
