Random action terminal exiting and redirecting to new thread

What issue are you seeing?

This might be two issues, but it feels like it happens at the same time. I haven't been able to detect a pattern, yet.

Basically, I can run one or more actions (launching a dotnet 10 API and a vue3 app, mostly), and after X amount of time, I'll come back to find that the terminal Window is gone and the application is no longer running.

The other possibly related thing is that I sometimes come back to find that I'm looking at a new thread. No prompt. A mix of previous settings and default settings.

Then again, I've also seen Codex freeze up a few times over the past couple of days, so maybe that's a third thing.

Close the issue if it's trash, or let me know what I can do to improve it.

What steps can reproduce the bug?

Honestly, I haven't seen these things happen in real time because I prompt and walk away. I'm still looking for a pattern, but I've seen it enough times that I just came here to read every issue title over the past 48 hours. I didn't find anything that matched what I was experiencing, so here we are.

What is the expected behavior?

If I run an action, I want it to run until I tell it to stop. And if it fails for some reason, at least let me know. Maybe the actions are failing and the terminal closes automatically as a response?

And if I'm looking at a thread when I walk away from my computer, I want that thread to be on my computer when I return. I don't want to admit how many threads I juggle sometimes :) , but it can be hard remembering which session I left off on.
