Permissions button almost always get stuck in frozen, animated glowing state when restoring older codex conversation in VSCode extension. The permissions level then can't be changed and the animation is extremely distracting.

Sometimes, the conversation itself won't load.

What steps can reproduce the bug?

1. Using Codex extension for VSCode, start a new conversation
2. Close VSCode and reopen it
3. Reopen one of the previous conversations in Codex history

Result: The permission button is stuck, frozen, in animated state

What is the expected behavior?

The permissions button is not stuck, not animated, and can be used.
