What version of the Codex App are you using (From “About Codex” dialog)?

26.421.11020

What subscription do you have?

Pro

What platform is your computer?

Windows 11 x64

What issue are you seeing?

Codex Desktop App for Windows to stop constantly prompting, even in Full Access mode. Codex promises to behave and use approved scripts to do its tasks and reverts to the old bad habits a minute later, and keeps reverting over and over. Codex even acknowledges it made a mistake and promises to not use ad-hoc commands that require approval, but it keeps reverting.

What steps can reproduce the bug?

Nothing seems to work. I can try to beg Codex to stop doing it and it promises to, but it reverts.

What is the expected behavior?

What do you mean? Run /feedback in Codex? I had an ID when I submitted, but I didn't copy it.

Yes, please repro the problem and then type /feedback in codex. It will upload logs and session details and then give you a thread ID that you can paste below. This will allow us to investigate and diagnose the problem you're seeing.

The next time I run into this problem in a session, which will be soon and often, I will do this and reply.
