Issue Title: Persistent flickering of loading icon and login failure
Description:
I am experiencing a persistent UI glitch where a white dashed loading circle keeps flickering in the center of the Visual Studio Code editor interface. Additionally, the Codex / Claude Code extension appears to be unable to complete the login process or load its primary interface.

Environment:

OS: macOS

Editor: Visual Studio Code

Extension: Codex

Steps to Reproduce:

Open Visual Studio Code.

The flickering loading icon appears immediately in the center of the screen.

Attempting to click on the extension icon or login does not resolve the issue; the extension remains stuck or fails to authenticate.

Expected Behavior:
The editor interface should be stable without flickering elements, and the extension should allow for a successful login and display its functional sidebar.

Actual Behavior:
A persistent flickering loading animation is visible (as shown in the attached video), and the extension functionality is inaccessible.
