This typically means that you have multiple copies of the CLI installed on your system. Perhaps one installed via npm and one by brew? Or one installed per-user and one installed globally? Look for the extra copy and uninstall it.

here is the issue:

brew upgrade --cask codex

This is assuming the user did run brew update... if not, brew upgrades to the last KNOWN version on the local system and it does not match what codex expects as latest version.

Solution:

change:

brew upgrade --cask codex

into:

brew update; brew upgrade --cask codex
