Session fails to load from time to time

What issue are you seeing?
Session chat is not loaded sometimes - an empty chat window is shown instead, while opening other chats works normally. After restarting the VSCode window, the same chat can load normally, or it may not load again. In that case, the only reliable way to make it load is to reload the VSCode window again, then first open another chat - it usually fails to load - then open the desired chat - it will now load (while the other one will not unless you do this sequence for that chat).

What steps can reproduce the bug?
- Use VSCode Codex extension
- Have a few chats
- Observe the described behavior from time to time
What is the expected behavior?
Chats loaded normally in ALL cases when we opened them

This is confirmed not to be an issue with my local setup - a friend of mine has exactly the same problem with the same "workaround" path described above, on a completely unrelated project/setup.

It is frustrating that these issues keep appearing and OpenAI has such weak QA for one of its main products.

This is still happening for me in 26.506.31421. I have found a way that usually makes the chat load: Open the same chat in the Codex App while the extension is trying to load it. When it loads in the Codex App, suddenly the extension finishes loading and displays it too.
I reproduced similar intermittent Codex chat-loading failures on Linux with VS Code 1.128.0 and Codex extension 26.707.31428.
Normally, the Codex loader animation starts and then freezes while the rest of VS Code remains responsive. Reinstalling, changing
extension versions, and using fresh user data did not reliably solve it.
Launching VS Code with GPU acceleration disabled fixes the issue:

This suggests that at least some reports categorized as session/history-loading failures may actually be Electron/Chromium GPU
rendering failures in the Codex webview. It would be useful for other affected
