Session fails to load from time to time

What issue are you seeing?

Session chat is not loaded sometimes - an empty chat window is shown instead, while opening other chats works normally. After restarting the VSCode window, the same chat can load normally, or it may not load again. In that case, the only reliable way to make it load is to reload the VSCode window again, then first open another chat - it usually fails to load - then open the desired chat - it will now load (while the other one will not unless you do this sequence for that chat).

What steps can reproduce the bug?

1. Use VSCode Codex extension
2. Have a few chats
3. Observe the described behavior from time to time

What is the expected behavior?

Chats loaded normally in ALL cases when we opened them

Additional information

This is confirmed not to be an issue with my local setup - a friend of mine has exactly the same problem with the same "workaround" path described above, on a completely unrelated project/setup.

It is frustrating that these issues keep appearing and OpenAI has such weak QA for one of its main products.
