Codex App asks for permission for every single edit per session

No matter how many times I _ONLY_ select "Yes, and don't ask again this session" for file edits, Codex still asks me again for every single file edit (in the same session)

This happens in all modes: Default permissions, Auto-review, Custom (config.toml)

It doesn't matter which I select, I still get asked to approve every single file edit

What steps can reproduce the bug?

brew install --cask codex-app log in to the app start using codex
