Review mode hides inline ::code-comment findings

What issue are you seeing?

In Codex app Review mode, it says it found actionable issues, but the inline review comments are not visible in the conversation. The response appears to omit the actual ::code-comment{...} directives/findings.

Codex is aware of the findings and can output them as text, but not as the cards.

What steps can reproduce the bug?

1. Ask Codex: “Please review my uncommitted changes.”
2. Codex responds with “I found a few actionable runtime issues...” and findings are not visible
