Codex Web loses track of commits, state desyncs, stops suggesting to update PR or create new, does not show changes, not able to push

What issue are you seeing?

Codex made 2 commits
They are not reflected in the UI nor in github branch, they are local to codex VM
UI does not show the changes nor suggests to update/recreate PR

What steps can reproduce the bug?

I created a PR at some point and requested additional changes, then I stopped one of the runs after it made a commit when I asked for a read-only answer, after it the changes stopped appearing
It also happened again in different session
It completely lost changes it made in that session at some point

What is the expected behavior?

UI should reflect changes
UI should suggest to sync
Codex should be able to push to its own branch on github
