Skill names don't show in Codex App when auto-used

What issue are you seeing?

When codex decides to use a skill on its own, it should mention the name of the skill. I have no idea what it's reading or using:

What steps can reproduce the bug?

Give codex some work that may trigger it to use an existing skill on its own.

What is the expected behavior?

Should always mention the skill used.
