Codex stream disconnected before completion on clean conversations

What issue are you seeing?

Problem:
Codex repeatedly fails with reconnect loops and never completes the response.

Observed messages:

Reconnecting... 1/5
Reconnecting... 5/5
stream disconnected before completion
error sending request for url (https://chatgpt.com/backend-api/codex/responses)

What steps can reproduce the bug?

Just send a prompt

What is the expected behavior?

Happens even in completely new conversations
Happens with relatively small prompts
Renaming/removing AGENTS.md did not solve it
Internet connection remains stable
Reproducible consistently

Additional information

Codex should either recover the stream or fail gracefully instead of looping indefinitely.
