Codex App: @ plugin picker stuck on 'Loading plugins...' after latest update

What version of the Codex App are you using (From “About Codex” dialog)?

What subscription do you have?

What platform is your computer?

Darwin 25.4.0 arm64 arm
macOS 26.4.1 (25E253)

What issue are you seeing?

In the Codex App composer, typing @ no longer shows or searches plugins. The picker opens, shows:

Plugins
Loading plugins...

Files
Type to search for files

Then the plugin area disappears / remains unusable. Plugin mentions that used to resolve before the latest app update no longer resolve from the composer.

What steps can reproduce the bug?

  1. Open Codex App on macOS.
  2. Open any thread/composer.
  3. Type @ in the composer.
  4. Observe the mention picker.

Observed result: the picker gets stuck at Loading plugins... for plugins, then the plugin suggestions disappear. File search still appears below it.

I restarted Codex and the issue persisted. I also reproduced the same behavior on another computer after the latest Codex App update, which suggests this is not specific to one local plugin cache/config.

Local diagnostics from this machine:

Codex App: 26.513.20950 (build 2816)
Codex CLI: 0.131.0-alpha.9
Platform: Darwin 25.4.0 arm64 arm
macOS: 26.4.1 (25E253)

App server process was also observed using elevated CPU while the plugin picker was failing:

/Applications/Codex.app/Contents/Resources/codex app-server --analytics-default-enabled

What is the expected behavior?

Typing @ should show searchable plugin suggestions, including installed plugins such as superpowers, like it did before the latest Codex App update.

Additional information

I initially suspected local plugin metadata/config, but after reverting local changes and reproducing the same behavior on another computer with the latest Codex App, this looks like a regression in the app/plugin mention picker path.

Existing issue search did not show an exact match for @ plugin picker stuck on Loading plugins... on macOS.
