Codex Desktop macOS plugin UI broken for installed plugins

What version of the Codex App are you using (From “About Codex” dialog)?

26.513.20950 (2816)

What subscription do you have?

Pro

What platform is your computer?

Darwin 25.3.0 arm64 arm

What issue are you seeing?

Problem:
Installed plugins page is broken. Every installed plugin detail page opens to a blank/black screen. In the composer, @ mention autocomplete shows “Loading plugins…” and never lists installed plugins.

Local plugin state seems OK:

  • Superpowers skills load in the composer
  • Direct plugin links work, for example:

plugin://superpowers@openai-curated

  • plugins are present locally and enabled in ~/.codex/config.toml

Relevant logs:
method=plugin/list
https://chatgpt.com/backend-api/ps/plugins/installed?scope=WORKSPACE
404 Not Found: {"detail":"Not Found"}

Earlier I also saw one:
403 Forbidden with Cloudflare HTML challenge:
“Enable JavaScript and cookies to continue”

Tried already:

  • logout/login
  • app restart
  • full computer restart
  • different internet connection
  • clearing Electron UI cache/local storage
  • reinstalling latest Codex DMG from official Codex page
  • verified latest official DMG still reports Codex Desktop 26.513.20950

This persists only on my account/app state. Another user's Codex plugin UI works.

What steps can reproduce the bug?

It never shows Superpowers or any other installed plugin like Slack.

Any installed plugin details page is like that.

What is the expected behavior?

Show the plugins.
